    Plaquenil toxicity bull's eye maculopathy

Her rheumatologist instructed her to undergo a baseline ocular examination prior to initiating Plaquenil (hydroxychloroquine, Sanofi-Aventis) therapy. Toxic maculopathy associated with chloroquine use was first documented in the literature five decades ago.1 In the United States, Plaquenil––an analog to chloroquine––is used to treat a variety of conditions, including rheumatoid arthritis, lupus and several distinct inflammatory disorders. Although the incidence of macular toxicity is infrequent with Plaquenil use (at a dosage of 200mg or 400mg q.d.), its visual impact can be devastating.

These findings were consistent with bull's eye maculopathy, a classic feature of advanced hydroxychloroquine toxicity. 3 months after medication was stopped, the patient's visual acuity had remained stable, and no other complaints were reported. On examination, a telltale sign of hydroxychloroquine toxicity is a bilateral change in the retinal pigment epithelium of the macula that gives the commonly described appearance of a bull’s-eye. This is a late finding, however, and too late for screening to be useful. Hydroxychloroquine, sold under the brand name Plaquenil Sanofi-Aventis, is an antimalarial drug that has gained widespread use in treating various autoimmune diseases, including systemic lupus erythematosus and rheumatoid arthritis. 1 By some estimates, more than 150,000 patients are on long-term therapy with this medication in America alone. 2 Retinal toxicity associated with HCQ use is.

Initially, central visual acuity may be unaffected, but the patient may notice related paracentral scotomas that often interfere with reading. The associated classic retinal toxicity is described as a bull’s eye maculopathy (ring of depigmented retinal pigment epithelium that spares the foveal area).

Prednisone/Plaquenil question - Prednisone/Plaquenil question. Support Forums. Plaquenil is a great drug I have no experience with it, but many people with lupus take it. Both pred and plaq are often prescribed together. I'd ask your rheumy about it and have them explain why they made that choice for you.

Plaquenil side effects - kidney problems? - Symptoms - Life With Lupus Water Retention/ Weight Gain; In summary Plaquenil isn't causing tyour new problems. It probably caused only the nausea, diarrhea, stomach cramps, and intense vertigo. BTW 100 mg daily may reduce side effects, but this low dosage also greatly annihilates its positive effects. 100 mg are a drop maybe two. on a hot stone. And finally
