We report two cases of hydroxychloroquine-induced hyperpigmentation presenting in a 50-year-old Caucasian female case 1 and a 78-year-old female case 2, both receiving 400 mg per day. Drug-induced hyperpigmentation is a relatively frequent reason for consultation, especially in polypharmacy patients. The sample may have been biased as many patients receiving treatments frequently associated with drug-induced hyperpigmentation, such as antineoplastic drugs, are diagnosed and treated by other specialties, such as oncologists. Demonstrated that hydroxychloroquine-induced pigmentation is not rare. The imputability of hydroxychloroquine in the genesis of this discoloration is difficult to establish. Our study supports the hypothesis that ecchymosis, platelet antiaggregants and oral anticoagulants may be the main predisposing factors to hydroxychloroquine-induced.

On examination, a telltale sign of hydroxychloroquine toxicity is a bilateral change in the retinal pigment epithelium of the macula that gives the commonly described appearance of a bull’s-eye. This is a late finding, however, and too late for screening to be useful. An elderly woman presented with 6 months history of skin coloured to erythematous papules in annular pattern over extremities, upper back and neck. The patient was diagnosed with granuloma annulare GA and started on hydroxychloroquine HCQ 300 mg daily. After 4 months of treatment, she developed bluish grey pigmentation within the plaques of GA with normal surrounding skin figure 1A,B.
